AB1217 CHRONIC LOW BACK PAIN AND MUSCULAR HYDATIDOSIS: THE TREE THAT HIDES THE FOREST

Background Hydatid cyst disease is a zoonosis caused by Echinococcus granulosus . The parasite implants most commonly in the liver and lung. Musculoskeletal involvement rare. Objectives To describe clinical imaging presentation of spinal muscular hydatidosis. Methods We report two cases chronic low back pain hydatid cyst. Results Case 1: A 36-year-old woman living rural area presented with three-years history lumbar complicated right-sided sciatica intermittent episodes urinary retention. was gradually worsening resisting conventional analgesics physiotherapy. On physical examination, she had tenderness left sacroiliac joint limited mobility. Biological findings were normal X-ray features showed osteolytic lesions L5 side. magnetic resonance (MRI) results revealed septated multilocular hydatic cysts high signal intensity T2-weighted images T1-weighted along sacrum Serological test for positive. treatment wide surgical resection combined Albendazole (400 mg/day) one year postoperatively. 2: 62-year-old one-year gait instability right flank swelling. normal. patient first explored uro-CT to assess nephrolithiasis it retro sub-peritoneal (60x80mm, 36x35mm) iliopsoas adductor muscles. MRI same cystes described on CT (Figure 1). No other localizations detected. diagnosis hydatidosis confirmed perioperatively after visualization Wide performed but signs recurrence detected at 3-month follow-up.
